The Nurse Practitioner will be responsible for:
Assisting in building our growing Heart Center with our group of dedicated Case Coordinators who work with families, patient's primary care physicians, referring providers, corresponding departments, specialty clinic RNs and/or medical staff to: develop in-take inquires, triage emails/calls, coordinate patient access, and help inform and update clinical communications.
Building programmatic clinical infrastructure in conjunction with Heart Center Operations and Nursing leadership.
Providing oversight and coordination of care of cardiovascular patients.
Facilitating Intake process by providing comprehensive clinical oversight.
Designing and implementing family centered systems that will enhance the patient experience and provide necessary information to clinicians to optimize care across the cardiovascular continuum of care.
Reviewing clinical itinerary for all complex cases (complex= 3 or more program/department/division consults).
Assisting in navigating any questionable referrals to the appropriate Heart Center team, clinic or specialty program. The nurse practitioner will be alerted to urgent concerns of the families and/or the referring clinicians that require review; and will assess, analyze and plan appropriate interventions, while collaborating among disciplines.
To qualify, you must have:
A Master's in Nursing (FNP/PNP).
Advance practice experience assisting in the care coordination of patients with complex medical needs.
5+ years of pediatric RN experience required.
2-3 years of nursing at Boston Children's Hospital strongly preferred.
NP experience strongly preferred, but will consider new NP's with the above experience.
Experience managing pediatric complex care, and/or outpatient clinic experience preferred. Strong leadership and communication skills required. Program management and supervisory skills preferred.
Must be highly organized with strong written and verbal skills.
